Delegate Accreditation
All persons participating in CHOGM must be accredited by the National Secretariat for CHOGM 2009 through the procedures set out at (c) and (d) below. A personal, non-transferable Identification (ID) badge will be issued to each person participating in CHOGM. This ID badge must be worn at all times and visibly displayed in order to access official CHOGM sites. Delegations should note that different levels of access will be granted to CHOGM venues and events.
- Delegates: Official delegates and spouses of member countries; I.G.O’s and Special Guests.
- Ancillary Staff: Support staff attached to the Heads of Delegation and spouses; Ministers of Government and spouses and other Senior Delegates of member countries.
In order to guarantee accreditation, official delegates intending to stay at official hotels (The HYATT Regency and the Hilton Hotel Trinidad) should submit theirAccreditation and Accommodation Forms by 15 September, 2009. Delegates intending to stay aboard the Cruise Ships will be accredited and should submit their Accreditation and Accommodation forms by 15 September, 2009. All other participants must register prior to 15 October 2009. Registration forms received after the stipulated deadlines may be subject to processing delays and the National Secretariat cannot guarantee that accreditation will be completed in time for CHOGM.
On-line registration will be available from 31 July 2009. The National Secretariat for CHOGM 2009 will provide each Delegation Secretary with the website address, username and password required to register all participants from his/her Delegation. The registration of Delegates must be authorised by the Delegation Secretary in order for accreditation to proceed.
The following mandatory fields must be completed in order to save or print your form.
- Surname
- Given Name
- Name as you wish it to appear on the pass
- Address (last 12 months, maximum two addresses
- Employer (last 12 months, maximum two employers)
- Date of Birth
- Country of birth
- Identification document information (Type of document, ID number, Country of issue, Expiry date)
The Delegation Secretary will be required to provide the NationalSecretariat with the following:
- Completed registration form for each delegate with a digital photograph
- Arrival/Departure schedules
- Name and designation of members of the delegation in order of precedence
Photograph submissions must comply with the specifications listed below:
- JPEG format, no larger than 50KB
- Full colour and passport sized (50 mm x 70 mm)
- Photograph should be taken against a pale or white background
- Taken in the past six months
- File name for digital images must be identified as follows: surname, given name and date of birth (Year/Month/Day)
- Any printed photographs must be identified on the reverse with the surname, given name and date of birth (Year/Month/Day)
